Please complete this worksheet and return it to our office by email, mail or fax. Then contact us to schedule an appointment for an individual consultation.

If you prefer to use email, please save your data in this PDF form with Adobe Reader 8 or newer, attach the file in your email program, and email it.

Childhood
For some parents, talking to a child about money is almost as hard as broaching some of those other difficult subjects. But understanding what money is and how to manage it is a crucial life skill parents must pass on to their children. It’s never too early to start giving your child a solid foundation for making a lifetime of financial decisions.
College
College is a time of new found freedom for many students. But that can spell trouble if that freedom applies to personal finances too. Students need to understand basic money management skills such as living within a budget and handling credit and debt. A solid financial foundation can lead to a lifetime of financial success.
Career
Money management should never get in the way of career advancement. Learn what you need to know to take advantage of a wide range of employer benefits, prepare for unexpected career changes and chart a clear path to job success.
Military & Reserves
Members of our nation’s Armed Forces are trained to defend our country in a moment’s notice. But like non-Service members, they have to take care of the homefront too. Understanding budgeting, saving and investing is a first step toward financial freedom. Start now so that you’re financially ready too!
Couples & Marriage
Compatibility means a lot more than liking the same movies or flavor of ice cream. It also means being able to successfully talk about and manage your finances. Begin now so that you can merge your money and your life.
Parenthood
When your friends said “a baby changes everything,” you didn’t know they meant your finances too! A baby brings a whole new set of financial priorities, from budgeting for the additional cost of raising a child to finding affordable child care to saving for a college education. It’s all possible with some planning.
Home Ownership
For many, the American dream still means owning a home. Whether you’re buying for the first time, refinancing or beginning home improvements, you need to understand the complex issues involved.
Entrepreneurs
Whether you’re starting a business, building the one you have, or stepping up to run a family business, you need a lot of information to help you make sound financial decisions. You’re not alone—millions of small businesses contribute every day to keeping America’s economy strong. Find out what you need to know to keep your business strong too.
Life Crisis
Sometimes the unexpected happens. You lose a job, your home is devastated by fire, an illness affects a loved one. Learn more about how to manage your finances during life’s difficult times. And start planning now so you’ll be prepared.
Caring for the Elderly
How do you care for your children and your parents at the same time? It’s a challenge millions of Americans are currently facing. Begin by examining your budget and financial goals.
Retirement
More than ever before, Americans need to take responsibility for saving for retirement. Whichever savings vehicles you choose—from 401 (k) plans to mutual funds to annuities—it’s never too early to begin preparing you for your Golden Years.
Financial Planning for Women
Women today control individual and family spending, run the majority of small businesses in the country and an increasing number of corporations, select their own investments—and need solid information about personal and business finance. AICPA created 360 Degrees of Financial Literacy for Women to answer those questions directly, professionally and simply with FAQs, articles and interactive tools for every aspect of money management and financial planning.

Making Home Affordable
The Obama Administration has introduced a comprehensive Financial Stability Plan to address the key problems at the heart of the current crisis and get our economy back on track. A critical piece of that effort is Making Home Affordable, a plan to stabilize our housing market and help up to 7 to 9 million Americans reduce their monthly mortgage payments to more affordable levels. The Home Affordable Refinance Program gives up to 4 to 5 million homeowners with loans owned or guaranteed by Fannie Mae or Freddie Mac an opportunity to refinance into more affordable monthly payments. The Home Affordable Modification Program commits $75 billion to keep up to 3 to 4 million Americans in their homes by preventing avoidable foreclosures. Our consumer website, www.MakingHomeAffordable.gov, provides homeowners with detailed information about these programs along with self-assessment tools and calculators to empower borrowers with the resources they need to determine whether they might be eligible for a modification or a refinance under the Administration's program. Through this website, borrowers can also connect with free counseling resources to help with outstanding questions; locate homeowner events in their communities; find a handy checklist of key documents and materials to have ready when making that important call to their servicer as well as FAQs from borrowers in similar circumstances; and much more.
